| Type | Defaults to the Default document type and to change, select from the drop down list |
| Description | Information the describes the document |
| Filename | Select from the search window attached to the field to select for this record |
| Last Modified Date | System date and time the document was added or the line changed |
| Modified By | Jiwa user who added the document or changed the line details |
